Recovery &rehabilitation.

“I’ve just left hospital and I’m not sure what to do next.”

After a hospital stay, you may need to arrange rehabilitation, home support and follow-up appointments. We help coordinate these, so you and your family have clearer next steps and continuity of care.

Illustrative scenario

Recovery after a stroke, from hospital to home.

After a stroke, a patient may need rehabilitation, physiotherapy, home support and specialist follow-up after leaving hospital. Their family may be managing these arrangements alongside work and other responsibilities.

At discharge

Organise discharge information and note agreed follow-up appointments and practical next steps.

In the first weeks

Help identify rehabilitation, physiotherapy and homecare options, then coordinate agreed services and appointments.

Ongoing

Track agreed follow-ups, share updates with the authorised family contact and check whether coordination needs have changed.

This is an anonymised, illustrative scenario, not a real client story. Every journey is different.

All FAQs
Can you help before discharge?

Yes. If you contact us before discharge, we can help organise practical arrangements and follow-up. We can also support you later in recovery from the non-clinical side.

Do you provide rehabilitation?

No. AA Health Central’s Patient & Family Journey Support service is non-clinical and doesn’t provide rehabilitation or therapy. We can help identify providers and coordinate agreed appointments and services.

How often will you check in?

We’ll agree the frequency of check-ins with you based on your support needs. We can review it if those needs change.
