What counts as a “complex” journey?

Care may feel complex when it involves several providers, multiple conditions, surgery or a complex treatment pathway, or coordination among family members. If you’re unsure, we can talk through your situation together.

Do I need an assessment first?

We start with a conversation about what’s happening and what support you may need. If a more detailed assessment would help, we’ll explain the next steps and recommend a support plan based on your situation.

Can you help with ongoing chronic conditions?

Yes. We can provide ongoing, non-clinical support with appointments, records and follow-up across providers. Your healthcare team remains responsible for medical advice and treatment decisions.
